Independent Investigation for a Rail Infrastructure Manager

We were appointed as an independent investigator to examine a safety‑related operational incident for a national rail infrastructure manager. The work involved reviewing how changes to operational equipment and infrastructure had interacted with day‑to‑day railway duties, including the technical integration of signalling, platform‑based systems and supporting operational processes. Through document review, site observations and interviews with staff, we identified the sequence of events and clarified the technical and organisational factors that had contributed to the issue. Our investigation provided the client with clear, practical recommendations to strengthen their engineering management, assurance processes and operational communication.
